Patient Cancellation Policy Reminder:
In order to maintain the schedule of our practice, we require a 24 hour cancellation notice of appointments. Failure to notify our office of cancellation will result in a $10 fee payable by the patient at the next scheduled appointment with the Psychiatrist, MD or Psychiatric Nurse Practitioner.
